I would like to put my Y membership on hold for up to 3 months at $10/month. Membership Holds require a 5-day notice prior to your draft day.

I would like to add or remove people from my Y membership, and/or change my membership type, and/or remove any membership add-ons from my account. Membership Changes require a 5-day notice prior to your draft day.

I would like to end my membership with the Y.

I understand that if I cancel my membership, I may be charged non-member rates for programs my unit is enrolled in, if applicable.

I understand for my membership to be cancelled for the following month, I need to cancel prior to the final day of the current month, regardless of which day I am drafted.

I understand it is my responsibility to verify that the cancellation was successful, and the Y is no longer taking membership fees out of my account. I understand that the Y is not responsible for reimbursing membership fees for more than (1) one month after my date of cancellation request.
